Learning from incidents with Toolbox

"Not surprisingly, companies usually prefer privacy about incidents that impact their safety records," says Charly. "They’re understandably reticent about opening up to rivals because of the possible commercial or legal implications – even though every safety professional recognises that the best way to learn is from real-life events. After all, almost every incident will have already been experienced by somebody, somewhere."
Developed by the Energy Institute (EI), Toolbox is a simple, engaging web app that provides health and safety insights from leading energy companies including BP, Chevron, ExxonMobil, Repsol and Shell. Funded by a grant from Lloyd’s Register Foundation, Toolbox uses stories about incidents and accidents to remind workers of how to work safely. Its genius is that all details are anonymised, which encourages people to upload full and frank details of safety-related incidents they’ve experienced.
Available in ten languages to encourage global take-up, the app features 16 work activities such as commissioning, process operations and construction as well as ten high-risk situations. For each area, people can drill down into dozens of real-life case studies, with each one described under the same four practical headings: What happened? Why did it happen? What did they learn? And practical advice so that users or their crew can learn from the incident and avoid repeating it.
"We’ve submitted a number of stories ourselves about incidents at Repsol," adds Charly. "By putting the inside story of an incident right there at our fingertips, Toolbox really helps us learn from each other and keep more people safe at work."
